For the convenience and privacy of our patients, Paradise Valley Foot & Ankle has made our most common patient forms available for download. If at all possible, please download and complete these forms in the comfort and privacy of your home prior to your appointment and bring them with you to your visit. Please arrive 20 minutes early and bring your insurance card, photo identification card, and a referral if required by your insurance. Please bring an updated medication list and any MRIs or x-rays . All co-payments and account balances are due at the time of your visit. We accept the following as form of payment; cash, checks,Visa, Mastercard, American Express, and Discover credit cards.
We participate in most HMOs, PPOs, and private insurance plans. Please contact our office for more information on our participation in your particular plan. Co-payments and account balances are due the time of your visit. Paradise Valley Foot & Ankle accepts all payments made with check, cash, Visa, Mastercard, American Express, or Discover. You may pay in person, by mail, or over the phone during our business hours.
Workers’ Compensation is a service provided by your employer if you have, unfortunately, sustained an injury while on the job. Paradise Valley Foot & Ankle is proud to accept patients who have suffered an injury and require treatment.
Our physicians are capable of handling trauma sustained below the knee including fractures, contusions, musculoskeletal trauma such as tendon tears, tendonitis, puncture wounds, crush injuries, and other injuries that may occur to the foot and ankle. If your injury requires surgery, our physicians have privileges throughout the Phoenix and Scottsdale region at multiple facilities to appropriately address your surgical needs.

URGENT CARE
If you have, unfortunately, sustained a foot or ankle injury which requires medical attention, our office holds available appointments for same day evaluation. We understand Emergency Room co-pays can be expensive and wait times can be exhaustive. We offer our urgent care service to expedite your diagnosis, treatment, and recovery. If your insurance plan requires a referral to visit a medical specialist, please obtain such a referral prior to your appointment.
